ACTIVITY FORMS DURING REMOTE INSTRUCTION
Please note that clubs/organizations must be ratified through the Inter-Club Council (ICC) to host official events, meetings, etc. All the forms listed will be need when planning virtual club/organization related events. Forms can be downloaded, electronically filled out, and emailed to the ICC advisor at Student_Activities@sac.edu once completed.
ACTIVITY APPROVAL/PERMIT FOR USE OF FACILITIES
This form will be used for ALL virtual club related activities, including the ICC funding proposal form found below. Please allow at least three weeks prior to your event date for processing. Additional forms are to be used depending on the event.
ICC FUNDING PROPOSAL FORM
If your club/organization is proposing for ICC funding, this form, club budget balance, activity approval form (above), and flyers/invoices must be turned in 2 weeks prior to the event. See ICC Constitution for more information.
SAC CONFERENCE REQUEST FORM
If your club/organization is attending a virtual conference, this entire packet must be filled out by your advisor and submitted to ICC advisor three weeks prior to the conference.
REQUEST FOR CHECK/CASH ADVANCE
Used to pay event costs with funds in your SBO Club Account and access your for funding.

This form must be completed by the club/organization OR every student participant that attends a virtual event/conference.

- REVENUE POTENTIAL/FUNDRAISING RECAP
Planning a fundraiser? Authorization form must be submitted prior to the fundraiser. Revenue potential form is to be completed as a recap with 1) estimated amount of $$ before the event and 2) actual amount of $$ after the event.
