What: |
This form
provides information for the Internal Revenue Service and is
required for all
performer/guests who are being monetarily compensated for their
services. |

Who: |
The
individual(s) who are being paid for performing/appearing on campus,
must fill out this form so the sponsoring club can submit it along with
the necessary paperwork to the SBO and the Student Activities Office. |

When: |
This form
should be picked up at least 4 weeks prior to a scheduled event and
returned three weeks prior to a scheduled performance/appearance. |

Where: |
The W-9 Form
can be obtained at the Student Activities Office. |

In order for
the check request to be process this form must be submitted. |
