Student Activities Club Resource Manual Forms


VENDOR PACKET

What: Provides the necessary information and guidelines to bring a vendor to campus. 
Who: Clubs can sponsor vendors.
When: This packet should be picked up as soon as possible in order to ensure adequate amount of time for the vendor and club to submit the necessary & required documentation.  Three weeks are required for processing the complete vendor packet once it is submitted.  The complete vendor packet includes an Activity Approval Form, Certificate of Insurance for $1,000,000 in liability insurance and a copy of the State of California Seller's Permit.
Where: The Vendor Packet can be picked up in the Student Activities Office 
(U-121A). 
Why: In order to ensure that all documentation is received prior to a vendor's arrival; and to allow processing time for the Student Activities Office as well as the Facilities Department.
