VEHICLE DRIVER’S REPORT

What: |
The Vehicle
driver’s report form must be filled out when a faculty/staff members
drives a RSCCD vehicle to a pre-approved destination. |

Who: |
The
faculty/staff member designated to drive the RSCCD vehicle must complete
this form. |
|
When: |
The Vehicle
driver’s report should be picked up prior to departure. The completed
form must then be placed in an envelope and returned to Eloise Morales
in the Administrative Services Office, B-10. If a vehicle is returned
after 5:00pm or on weekends, the form must be placed in the white drop
box located on the maintenance yard gate across from the campus vehicles
parking lot. |

The Vehicle
Driver’s Report form and envelope can be picked up at the Student
Activities Office in U-121-A. |
| Why: |
This form
provides proof and documentation to the Rancho Santiago Community
College District Office of the vehicle # used, any credit card issued,
name and license number of the driver, name of club, destination,
departure time, date, mileage, vehicle damage, and fuel tank. |
