What: |
This form is
used for the approval of a faculty or staff member as a driver to any
campus related event. |

Who: |
Faculty and
staff advisors are required to submit this form for approval prior to
driving students to any event. |

When: |
This form
should be submitted to Risk Management Services at the District Office
as soon as possible. Although this request takes only 5 business days
to complete, it is recommended that every advisor submit this form for
approval at the start of the semester to allow for any last minute
driving needs. |

Where: |
This form can
be picked up in the Student Activities Office, U-121-A. |
| Why: |
To allow
faculty and staff to drive students to and from various events. This
form also provides documentation that the faculty/staff person filling
out the request agrees to comply with the District’s policies and the
State’s driving laws. |
