What: |
This form is
used to request a check or transfer of funds. |

Who: |
Any club with a
SBO Account can utilize this form. This form requires
the account name,
number, name of requestor, department, contact phone number, description
of services, the dollar amount, the club president signature and the
club advisor signature. |

When: |
It can be used
anytime a club needs to withdrawal funds from its account. |
|
Where: |
This form is
available in the Student Activities Office and must be
submitted for
approval. Once the form is signed, it is sent to the SBO. |
| Why: |
Ensure proper
approval of withdrawing funds as well as compliance for
use of funds. |
