The Petition for Club
Registration must be filled out and returned to the Student Activities
Office along with the ICC Guide Sheet once a year in the Fall semester.
This form indicates the club status, name of club, purpose/goals,
advisor(s) name and contact and the names and student ID's of all
members. |
| Who: |
Returning clubs
and/or new clubs must fill this form out and turn it in each fall
semester (or at the time a club would like to activate). Only new clubs
(clubs that were not active during the fall) are required to submit this
form by the third week of school in the Spring semester. |
| When: |
This form must be
turned in by the third week of the fall semester on or before 4:00 pm to
U-121-A. Again, only new clubs (clubs that were not active during the
fall) are required to submit this form in the Spring semester. |

Where: |
This form must be
time stamped and turned in to the Student Activities Office U-121-A. |
| Why: |
Turning in this form
provides documentation that the club, its advisors and members agree to
the required guidelines. In addition, this form is used to verify that
each member is an enrolled RSCCD Student |
