INTER-CLUB COUNCIL GUIDE SHEET

What: |
The ICC Guide Sheet provides the Student
Activities Office and the Inter-Club Council with the name of a
club, status, meeting date/time/place, the name(s) of the advisors
as well as the names, student ID's and contact numbers for the
club officers. |

Who: |
A club advisor or one of the club's executive
members can fill out this form. |
|
When: |
This form must be completed and returned to the
Student Activities Office U-121-A by the third week of school on
or before 4:00pm. This form must be turned in each semester in
order to update advisor(s) and club officers' information. |

Where: |
This form can be picked up at the Student
Activities Office U-121-A. When completed, it must be returned to
back to U-121-A. Make sure the document is time stamped prior to
turning in this form. |

Why: |
This form must be submitted on time in order for
the club to receive a vote of confidence (activation) by the
Inter-Club Council each semester. If this form is not turned in
each semester, the ICC will not vote to activate a club. In
addition, Inter-Club Council Representatives must be enrolled in
at least 6 units and possess a cumulative G.P.A. of 2.0 or
better. This sheet is used to verify enrollment and GPA.
Lastly, this form provides documentation of the student delegated
as the ICC Representative. In order for a club to remain active,
the club must have an ICC Representative present at all ICC
required meetings and events. |
