HOLD HARMLESS & INDEMNIFICATION AGREEMENT

What: Hold Harmless agreement allows clubs/organizations to bring performers/guests on campus during their events.  This form must be signed by the individual(s) who are planning to come to campus in order to cover them under insurance and liability guidelines prior to their scheduled performance/attendance. 
Who: The actual performer/guest must sign this form.
When: The signed form must be turned in WITH the Activity Approval/Permit for Use of Facilities for Approval form.
Where: The Hold Harmless Agreement form can be picked up at the Student Activities Office in U-121-A and must be turned in back to U-121-A with an Activity Approval/Permit for Use of Facilities Approval form. 
Why: This form ensures proper liability insurance coverage and is a legal agreement between the District and the performer/guest on campus.
