ACTIVITIES/FACILITIES REQUEST FORM

What: This form is used to schedule events and reserve facilities and/or equipment needed by a club/organization.  If special set up is required, it is suggested that each club/organization provide a detailed drawing/layout of the set up.
Who: Every club/organization planning an event must fill out this form. 
When: This form is due at least 3 weeks prior to the intended event. 
Where: The form can be picked up in U-121-A and must be returned back to U-121-A. 
Why: The timeline of 3 weeks allows for adequate scheduling of facilities and equipment.  In addition, turning in this form will decrease the chance of scheduling conflicts.  When the form is turned in, it is reviewed by the Student Activities Coordinator and the Director of Student Services.  If approved, it is signed and submitted to the Facilities Office for their approval.  If the Facilities Office approves the event, an email notification is sent back to the Student Activities.  At this point, the Student Activities Office confirms the event on the Event Calendar and notifies the club of approval. 

ATTENTION:  Simply submitting an Activity Approval/Permit for Use of Facilities form does not automatically approve the event.  If you do not hear back from the Student Activities Office, it is recommended that you contact the office for verification of event approval.
