You can hand deliver or mail your completed application with the required additional documentation (see below) to the following address:

Copies of the following documents MUST accompany your completed application

  • DD-214 Separation Report
  • 1040 Income Tax Form or other income verification (i.e. public assistance or complete attached Income Declaration - see application)
  • Transcripts - High School or most current college transcripts (if available)
  • Current Photo ID (i.e. driver license, service ID card)
