Please contact our office for guidance if you have questions about which form to use. Our forms are available in two formats:


Online Forms (using Dynamic Forms)

  • This is our preferred method of submission. You can complete, sign, and submit these forms electronically. If additional signatures are required, the form will automatically route itself to the appropriate person for signature request. You will be required to use your Self-Service account to access these forms. 

PDF Forms

  • PDF forms are available for you to download and print. You can submit completed forms (including any required signatures) in person or via email to admissions@sac.eduNote: Adobe Acrobat Reader Plug-in is required to view these files.
